What a catering order form should capture

The purpose of an order form is to arrive complete. Every blank a client can leave empty becomes a phone call, so the form should ask, once and in writing, for everything intake needs: contact details, the event date and serve time, headcount, delivery or pickup with the location, the items with quantities, dietary requirements, the payment method, and, for corporate clients, the PO number and cost center their finance team will expect on the invoice.
